Summery :

The McDonnell Douglas F-15 Eagle is an all-weather tactical fighter aircraft.
This F-15D (#86-0173) was assigned to 65th Aggressor Squadron, 57th Wing at Nellis AFB.
On July 30th 2008, it was participating in Red Flag 08-3 as part of three F-15s plus eight F-16s simulating Red Air tactics against a large force of 26 Blue Air aircraft.
Duaring the flight, it experienced a left yawing and upright flat spin. While it lost approximately 20,000 feet in the spin and had spun approximately 20 times. The pilot tried to recover but, it began a coordinated left roll with decreasing pitch angle. The pilot and the observer pilot ejected at 700 feet AGL, 70 degrees nose low and in a slight left bank.
The observer pilot, an exchange pilot from the Royal Air Force, survived with minor injuries while the pilot Lt. Col. Thomas "Moses" Bouley sustained fatal injuries with virtually no parachute inflation at the time of impact.
The cause of the mishap was not clear, but it may possibly the "right horizontal stabilator failure" or the "external wing tank fuel imbalance".
